PRN LPN Job Summary: TruHeart Home Health and Hospice is seeking a skilled and compassionate PRN Licensed Practical Nurse (LPN) to join our team in Tulsa, Oklahoma. As a PRN LPN, you will provide prescribed nursing care services to ill, injured, convalescent, and disabled patients in their homes, following a Plan of Care (POC) according to the patient's needs. This is a per diem position, with opportunities for flexible scheduling. Responsibilities: Provide comprehensive health and psychological evaluations, monitoring of patient condition, health promotion, and prevention coordination of services, and direct nursing care. Demonstrate knowledge and competency in performing baseline nursing skills as outlined in agency policies and clinical policies. Implement nursing care to patients in the home setting, working under the direct supervision of an RN/MD. Administer medications and treatments according to Agency policy, accepted standards of care, and home care guidelines. Assist with rehabilitation according to the medical care plan, encouraging patients to help themselves using their own capabilities. Identify and determine priority of patient needs/problems, anticipate emergencies, and report abnormalities to the RN/MD. Requirements: Current Oklahoma LPN license CPR certification Driver's license and auto insurance One or more years of experience in home health agency or hospital setting Competency assessments must be completed Graduate of an accredited school of nursing for LPN Ability to use critical thinking to resolve problems Strong communication and organizational skills Ability to work in a fast-paced environment with varying patient needs Ability to access patient homes, which may not be wheelchair accessible Hearing, eyesight, and physical dexterity sufficient to perform patient care Work Environment: Moderate noise level Inherent potential for exposure to bloodborne pathogens Moderate stress and emotional demands Expected to work in all kinds of surroundings with varying individuals Benefits: Competitive hourly rate Flexible scheduling opportunities Opportunities for professional growth and development #J-18808-Ljbffr
