Major Responsibilities Ensures and improves quality of care by facilitating patients and families’ right to receive quality, cost‑effective care in line with Advocate’s MVP of Stewardship. Utilizes appropriate resources to respond to situations that could negatively impact outcomes. Adapts practice to the latest standards based on evidence‑based literature, utilizing appropriate tests, measures, and interventions to achieve outcomes. Participates in quality/process improvement initiatives. Promotes efforts to reduce risk and improve patient safety. Consistently documents decisively to support reimbursement and regulatory requirements. Acts as an effective member of the patient care team to formulate an integrated approach to care in line with Advocate’s MVP of Partnership. Makes sound clinical decisions, caring for all patients including those with complex problems. Recognizes changes in clinical situations and evaluates and implements changes to the plan of care. Prioritizes and organizes patient care, ensuring clear hand‑off communication to achieve optimal outcomes, using techniques such as SBAR and 5P’s. Initiates referrals and discharge planning for assigned patients and actively coordinates care using an interdisciplinary approach (as appropriate based on employment status). Actively participates in interdisciplinary case conferences (as appropriate based on employment status). Demonstrates respect and understanding for peers and other clinical disciplines, serving effectively in the patient care team and following the Advocate MVP of Excellence. Participates in staff meetings, agency in‑services, and educational activities to enhance development and support organizational initiatives. Utilizes available resources to maintain current knowledge relevant to practice. Demonstrates effective communication, feedback, and conflict‑resolution skills, promoting collaboration among healthcare team members. Seeks opportunities to enhance knowledge, skill, and competency of self and other department members. Develops and maintains a restorative relationship with patients and families throughout the continuum, following Advocate’s MVP of Compassion. Incorporates individualized plans of care, including assessments of physical, psychosocial, comfort, and wellness needs. Uses AHHS Standards of Behavior in interactions with patients, associates, and physicians. Adapts communication styles to individual patient needs. Assesses patient and family readiness, involves them in goal‑setting, planning, and implementation of care, and seeks consultation and referrals as needed. Provides care that routinely exceeds patient and family expectations, as measured by survey instruments. Promotes patient/family knowledge and confidence to manage self‑care, disease processes, medications, procedures, and infection‑control prevention. Demonstrates behaviors that enhance productivity and understand the financial and economic conditions in the healthcare environment. Adheres to personnel and operational policies. Collaborates and plans patient care with others to maximize efficiencies and conserve resources. Monitors resource utilization to achieve excellent health outcomes for patients. Commits to meeting patients’ and families’ learning needs, applying a systematic process for optimal self‑care and quality of life, following Advocate’s MVP of Equality. Assesses readiness to learn and adapts approach as necessary. Provides and adapts appropriate resource materials, using health‑literacy assessments and principles. Licensure, Registration, And/or Certification Required Occupational Therapist license issued by the state of Illinois. Speech‑Language Pathology license issued by the state of Illinois. Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) – to be obtained within six months unless otherwise determined. A valid driver’s license issued by the Division of Motor Vehicles. Education/Experience Required Master's Degree in Occupational Therapy; licensed in the State of Illinois (or eligible for licensure). Must demonstrate compliance with state continuing education requirements, have one year of clinical experience or a strong spectrum of clinical affiliation experience, and have graduated from an Occupational Therapy curriculum accredited by the American Occupational Therapy Association. Knowledge, Skills & Abilities Required Basic computer proficiency (e.g., Microsoft Word, email) and ability to learn and use computer‑based scheduling and documentation systems. Professional communication skills, both verbal and written, with strong documentation proficiency. Good interpersonal, time‑management, and self‑organization skills. Proficient use of home‑based therapy equipment and assistive/adaptive devices (balls, bands, weights, dressing aids, etc.). Licensed as an Occupational Therapist in the State of Illinois. Physical Requirements And Working Conditions Extensive travel within geographic boundaries in all weather conditions and on all road types, including expressways. Work environment includes patient or family homes, assisted‑living facilities, etc. Requires full range of body motion: bending, sitting, standing, climbing stairs while carrying objects, pulling, pushing, reaching, carrying, and lifting. Must be able to lift/carry at least 25 pounds, with some duties requiring up to 35 pounds. Assists and maintains patient safety during ADL activities and transfers using standard pivot transfers, transfer boards, or Hoyer lifts. Provides treatment on standard beds in home environments. For patient lifts over 35 pounds or when the patient cannot assist, patient‑handling equipment is to be used, with at least one other associate when available; unique scenarios will be assessed case‑by‑case.
