Beth Israel Lahey Health at Home is seeking a Hospice RN Case Manager to plan and deliver patient care using the nursing process, collaborating with families and the interdisciplinary team to ensure comfort and quality at end of life. Requirements include an Associate's Degree or BSN, MA RN license, BLS certification, and at least 1 year of direct care nursing experience; EPIC experience preferred.
