Cottage Care Director at Severna Park

We are seeking a compassionate, experienced, and hands-on Cottage Care Director (CCD) to lead our Memory Care neighborhood. This is an opportunity for an LPN or CMT who combines strong clinical knowledge with a genuine passion for individuals living with Alzheimer's disease and other forms of cognitive impairment.

The Cottage Care Director serves as both a clinical resource and people leader , overseeing the memory cottage care team while helping create a residential environment centered around dignity, familiarity, engagement, safety, and individualized care.

Working closely with the Resident Care Director, Executive Director, families, medical providers, and interdisciplinary team , the CCD oversees resident assessments and care planning, medication management, staffing, training, regulatory compliance, and the overall delivery of Memory Care services.

We are looking for a leader who is comfortable being present and involved, developing their team, communicating with families, identifying changes in resident needs, and stepping in when additional support is needed.

What You'll Do

• Provide leadership and supervision to Medication Technicians, CNAs, and other Cottage Care associates.
• Recruit, hire, orient, train, coach, and manage Cottage Care staff.
• Monitor associate development and reinforce ongoing and state-required training.
• Partner with the Resident Care Director and Executive Director to ensure compliance with applicable state regulations and community standards.
• Develop a strong care team and residential environment that supports the overall Cottage Care philosophy.
• Complete resident assessments timely and train designated team members in appropriate assessment skills.
• Develop, implement, and continually evaluate individualized resident care plans based on each resident's needs, abilities, preferences, and routines.
• Direct and coordinate resident care needs in collaboration with the care team, medical providers, families, and other community department leaders.
• Monitor physician orders to ensure appropriate follow-through and completion.
• Audit Medication Administration Records (MARs), communicate medication changes with the pharmacy, and oversee all aspects of the medication administration program.
• Monitor changes in resident condition, falls, behaviors, and other clinical concerns and ensure appropriate interventions and communication occur.
• Conduct professional care plan meetings with families and serve as a licensed advocate for residents' needs.
• Create and coordinate meaningful person-centered activities and life-skills programming designed specifically for individuals living with cognitive impairment.
• Provide education, guidance, and support to associates and families regarding Alzheimer's disease, dementia, and changing resident needs.
• Participate as an active member of the community management team, including Manager on Duty coverage, staff meetings, community presentations, marketing initiatives, and community events.

What We're Looking For

The ideal Cottage Care Director understands that successful Memory Care requires both clinical expertise and emotional intelligence . You should be comfortable leading a team while remaining connected to residents and families and understand the unique social, psychological, recreational, and clinical needs associated with Alzheimer's disease and other forms of dementia.

You will bring knowledge and experience in:

• Alzheimer's disease, dementia, and cognitive impairment.
• Resident assessment and development of individualized Plans of Care.
• Medication administration procedures and MAR oversight.
• Recognition and evaluation of changes in resident condition.
• Diagnostic review of resident falls and appropriate interventions.
• Coordination and communication with physicians and other medical providers.
• Conducting professional care plan meetings with residents' families.
• Supervisory and management practices, including coaching, training, and accountability.
• Person-centered Memory Care programming and life-skills activities.
• Resources and services available to seniors.
• HIPAA and resident confidentiality.

Qualifications

• Must be 21 years of age or older.
• Active LPN license in good standing with the appropriate state licensing authority is highly preferred, Certified Med Technician Required.
• Previous experience or training in an equivalent setting preferred.
• Previous experience working with Alzheimer's disease, dementia, and Memory Care residents strongly preferred.
• Genuine understanding of and concern for the needs of seniors.
• Strong communication, leadership, organization, and critical-thinking skills.
• Ability to build trusted relationships with residents, families, associates, medical providers, and fellow community leaders.
• Must possess and maintain a valid driver's license and an acceptable Motor Vehicle Record (MVR) in accordance with Company Fleet Safety standards. Driving is an essential function of this position and a condition of employment.
• MVR reviews will be conducted at hire and periodically thereafter in accordance with Company policy. Failure to meet or maintain Company driving standards may result in removal from driving responsibilities and may impact continued employment eligibility for this position.
