Patient Navigator Specialist 1

Overview The Patient Navigator Specialist 1 supports cancer prevention, screening, education, and patient facilitation as part of the multidisciplinary care team. This role assists patients before, during, and after cancer treatment across the continuum of care and connects patients with clinical and supportive resources to address barriers to care. Responsibilities Provide one-on-one assistance to patients and navigate the health care system to ensure timely screening, diagnosis, treatment, and post-treatment cancer care and supportive services. Collaborate with health care professionals to maintain a climate of respect, dignity, ethical integrity, trust, and teamwork; participate in multidisciplinary teams to deliver safe, timely, efficient, effective, and equitable patient care. Assess and address patient needs by understanding roles within the care team to optimize health and wellness. Serve as a point of contact among providers, patients, and clinical caregivers; advocate for patients in dealing with cancer and treatment plans. Evaluate barriers to care, access, and quality indicators; identify resources for cancer patients/survivors and their families across medical, insurance, financial, and social domains. Assist patients in identifying administrative, structural, social, and practical issues to participate in decision-making and seek solutions (including participation in clinical trials) to empower self-navigation. Maintain knowledge of cancer screening, diagnosis, treatment, survivorship, and related physical, psychological, and social issues to ensure appropriate referrals. Maintain and coordinate up-to-date resources addressing Social Determinants of Health; collaborate with care team, social work, and PCRMs to address patient needs. Document encounters, barriers to care, and resources or referrals in the medical record; focus on facilitating timely access to care across the care continuum. Qualifications Bachelor’s degree in education, Public Health, or a related field with emphasis on case management or social work; relevant experience or an equivalent combination of education and experience is preferred. Experience working with diverse populations and in community-based and multicultural settings preferred. Ability to establish and maintain positive working relationships with internal and external partners; effective communication skills including active listening and appropriate use of language.
