Medical Social Worker

The Medical Social Worker (MSW) will provide direct patient care under the supervision of the Director of Nursing (DON), adhering to the guidelines set forth by the California Department of Public Health (CDPH) and Title 22 regulations. The MSW will perform duties for residents in Oak Hills Manor, LLC, a Congregate Living Health Facility (CLHF), ensuring that each individual's health needs are met with care, compassion, and professionalism. ESSENTIAL DUTIES AND RESPONSIBILITIES: Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public Collaborate closely with nursing, therapy, admissions, and administration to facilitate smooth transitions of care Adapt quickly to changing resident needs in a high-volume, short-term rehabilitation environment Will act as liaison and referral with psychiatric and psychological service providers as well as ancillary service provider Must be a team player with strong clinical skills Must be familiar with the laws, regulations, and guidelines governing personnel administration Must be able to communicate policies, procedures, regulations, reports, etc., to personnel, residents, family members, visitors, and government agencies/personnel Ensure timely and accurate completion of all social services sections in compliance with regulations Lead and coordinate care plan meetings, including initial, quarterly, annual, and discharge care plan conferences, ensuring required disciplines and family/resident participation Experience completing psychosocial evaluations, social histories, cognitive, mood and behavioral assessments Actively manage and coordinate discharge planning, including collaboration with IDT members, hospitals, families, and community resources Proactively address and resolve resident and family concerns, grievances, and psychosocial needs Provide psychosocial assessments, counseling, and support services as indicated Exercise discretion and independent judgement that is free from immediate supervision in relation to items with significant impact on the business All other job duties as assigned MINIMUM QUALIFICATIONS: Education: Master’s Degree, MSW required Experience: Minimum of one (3) year of MSW experience in a healthcare setting, preferably in long-term care, acute care, or a similar clinical environment Skills and Abilities: Strong clinical and assessment skills Ability to work in a fast-paced environment and manage multiple tasks efficiently Strong organizational, communication, and conflict-resolution skills Ability to prioritize tasks and meet regulatory timelines in a dynamic setting Proficient in using electronic health record (EHR) systems Knowledge of Title 22 regulations and other relevant state and federal healthcare laws Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents, staff, and visitors based on whatever maturity level at which they are currently functioning WORK ENVIRONMENT The position is based in a Congregate Living Health Facility (CLHF), and the nurse will be exposed to a variety of medical conditions, including infectious diseases, physical strain, and emotional stress. Must be willing to work various shifts, including evenings, weekends, and holidays, as required by the needs of the facility.
