Our facilities function as Skilled Nursing Facilities (SNFs) that offer:
- Long-term care: For residents needing ongoing skilled support and compassionate daily assistance.
- Short-term, sub-acute rehabilitation: For patients recovering from surgery, illness, or injury.
- Collaborative care: An interdisciplinary team approach involving nursing, therapy, and medical staff working in unison.
- Diverse patient population: Including individuals with orthopedic, neurological, cardiopulmonary, and post-acute needs.
The mix of patients may vary slightly by facility. As a therapy professional, you will play a crucial role in helping patients achieve functional improvements and enhance their quality of life in a supportive, patient-focused setting.
Position SummaryThe Speech Language Pathologist (SLP) delivers direct rehabilitation services in speech therapy to patients referred by physicians, adhering to all relevant laws, regulations, and organizational standards.
Education, Experience, and Licensing Requirements- Graduation from an accredited speech therapy program (minimum MS or MA in SLP).
- ASHA certification (CCCs) OR eligibility for a 9-month Clinical Fellowship Year (CFY) to obtain ASHA certification (CCCs) and/or meet ASHA credentialing criteria. Must maintain ASHA credentials once achieved.
- Current registration/licensure in the applicable state, with an active license in good standing throughout employment.
- One year of experience preferred; however, new graduates are encouraged to apply.
- CPR certification required upon hire or to be obtained during orientation, with maintenance of current certification throughout employment.
- Engage in community health initiatives/projects as appropriate.
- Proficient in Microsoft Word, Excel, and email communication.
- Collaborate with patients, families, and support departments to effectively address patient needs.
- Exhibit proper body mechanics at all times.
- Demonstrate the ability to make independent decisions when necessary.
- Knowledgeable about practices, procedures, and regulations governing operations in post-acute care facilities.
- Implement and interpret departmental programs, goals, objectives, policies, and procedures.
- Perform competently in all required areas, including patient rights, safety, and sanitation.
- Maintain professional relationships with colleagues, vendors, and others.
- Ensure confidentiality of all proprietary and sensitive information.
- Adhere to organizational policies, including those related to harassment and compliance.
- Exhibit integrity and professionalism by following the organization's Code of Conduct and completing mandatory compliance training.
- Establish, assess, and adjust realistic, measurable, timely, and functional goals.
- (SLP CCC only) Oversee and evaluate care provided by SLP CFYs.
- (SLP CFY only) Follow speech therapy treatment plans under the supervision of the SLP CCC, in accordance with federal and state guidelines.
- Document patient progress accurately and in a timely manner.
- Utilize therapy software effectively and accurately.
- Provide excellent customer service and maintain a positive attitude towards patients.
- Assist in the safe evacuation of patients when necessary.
- Demonstrate reliable and regular attendance.
- Employ reasoning skills and good judgment in all situations.
- Communicate and collaborate effectively within an interdisciplinary team.
- Engage in physical activities such as sitting, standing, bending, lifting, pushing, pulling, stooping, walking, reaching, and moving intermittently during work hours.
- Read, write, speak, and comprehend the English language.
