Your recruiter will provide the most up-to-date details during the hiring process._ **How you'll make an impact in this role** + **Orchestrate Comprehensive Care Plans:** Lead the implementation and continuous monitoring of personalized care strategies to ensure every patient reaches their recovery milestones. + **Champion Multi-Disciplinary Coordination:** Act as the central point of contact for the entire care team, synchronizing specialists and support staff to provide a seamless, holistic patient experience. + **Translate Clinical Orders into Action:** Execute physician and nursing directives with precision, ensuring that treatments are delivered safely and effectively. + **Facilitate Safe Transitions:** Evaluate and coordinate discharge needs early in the stay, working closely with the healthcare team to ensure patients have everything they need for a successful return home. **What minimum requirements you'll need** Licensure / Certification / Registration: + BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. + Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.
