Clay County Memorial Hospital is seeking a compassionate and dedicated Registered Nurse (RN) or Licensed Vocational Nurse (LVN) to provide skilled nursing services to patients in their homes on a visiting and intermittent basis for Home Health services.

The RN/LVN will provide quality, patient-centered care while working collaboratively with physicians, patients, families, caregivers, and other members of the home health team. The RN will provide professional nursing oversight, care planning, coordination, and supervision as required. The LVN will provide skilled nursing services under the direction of a physician and/or Registered Nurse.

Essential Duties and Responsibilities:

Provide skilled nursing care and services to patients in their homes.

Make scheduled home visits and assess patient needs.

Perform appropriate nursing assessments and clinical procedures based on the patient's condition and plan of care.

Monitor and document patient condition and report changes to the appropriate RN and/or physician.

Provide nursing treatments and procedures requiring specialized skills.

Administer medications and treatments as authorized and within the scope of licensure.

Perform or assist with procedures including vital signs, dressing changes, catheter care, injections, IV fluids and medications, enemas, irrigations, colostomy care, and other appropriate skilled nursing services.

Educate and counsel patients, families, and caregivers regarding disease processes, medications, treatments, self-care, and safety.

Maintain accurate and timely clinical documentation, including visit notes, progress notes, medication records, and other required documentation.

Complete required admission, recertification, resumption of care, transfer, and discharge documentation as applicable to the position.

Participate in patient case conferences and assist with discharge planning and coordination of services.

Assist with coordination of community resources and alternate care when a patient can no longer safely remain at home.

Maintain ongoing communication with physicians, patients, families, caregivers, and other healthcare professionals.

Follow physician orders and the established plan of care.

Participate in quality improvement activities, continuing education, emergency preparedness training, and public relations activities as required.

Maintain patient confidentiality in accordance with HIPAA requirements.

Follow infection control practices and appropriate precautions when providing direct patient care.

Rotate on-call coverage as necessary, including weekends and after-hours, to ensure appropriate patient coverage.

Perform other duties as assigned by the RN/Patient Care Coordinator, Director, or Administrator.

The RN may additionally be responsible for:

Conducting initial evaluation visits and determining patient appropriateness for home health services.

Developing and coordinating the Plan of Treatment for physician authorization.

Providing professional nursing services requiring RN-level assessment and clinical judgment.

Reviewing, updating, and evaluating the Plan of Treatment, Nursing Care Plan, Medication Sheet, and Home Health Aide Assignment Plan.

Supervising LVNs and other members of the patient care team as assigned.

Providing education and training to patients, families, caregivers, and home health staff.

Participating in supervisory visits and case conferences.

Reporting patient status and changes to physicians and coordinating follow-up as necessary.

Qualifications

RN Candidates:

Graduate of an accredited School of Professional Nursing.

Current Registered Nurse license in the State of Texas.

Current driver's license.

Current CPR certification.

Current knowledge of nursing principles, standards, and practices.

Ability to independently manage multiple priorities and provide skilled nursing care in the home setting.

Home health/community nursing experience preferred.

Medical-surgical nursing experience preferred.

Medicare and Medicaid experience preferred.

Must maintain continuing education requirements as required for licensure.

LVN Candidates:

Graduate of an accredited school of vocational nursing.

Current Licensed Vocational Nurse license in the State of Texas.

Current driver's license.

Current CPR certification.

Ability to work independently and effectively communicate with patients, families, caregivers, and healthcare professionals.

Home health or community health experience preferred.

Medical-surgical nursing experience preferred.

Must maintain continuing education requirements as required for licensure.

Physical and Other Requirements

Ability to walk, sit, and stand intermittently throughout the workday.

Ability to bend, stoop, and reach.

Ability to lift up to 50 pounds and occasionally assist with supporting or moving patients who may become weak or helpless.

Ability to cope with the mental and emotional demands of the position.

Adequate vision and hearing, with or without reasonable assistance, to perform required duties.

Ability to work independently with flexibility, integrity, and professionalism.

Ability to drive to patient homes.

Ability to maintain confidentiality and comply with HIPAA and hospital policies.

Ability to work effectively with patients, providers, personnel, families, and support agencies.
