Hospice Registered Nurse Case Manager

Elara Caring is seeking a Registered Nurse Case Manager to join our team and drive compassionate home-based care. You will assess patients and families, coordinate interdisciplinary care, and educate staff across settings while upholding our mission and care standards. You will report to the Clinical Team Manager and travel up to 50% to patient locations, facilities, and partner sites. A nursing license is required and hospice or home health experience is preferred. #J-18808-Ljbffr
