RN Clinical Intake Coordinator - Weekends

Saturdays and Sundays can be worked remotely. Your Impact Oversee all daily patient intake and referral operations Manage the full intake process, including insurance verification and authorization Ensure compliance with all state, federal, and payer requirements Coordinate timely patient admissions in accordance with company policies Build and maintain strong relationships with physicians, hospitals, and referral sources Ensure smooth patient transitions to home care, including care coordination and education Track and report referral data, trends, and performance metrics Promote and maintain a positive reputation in the community as a high‑quality provider Identify opportunities to improve services and patient access to care Maintain knowledge of contracts, payer sources, and community resources Our Commitment to You Perform meaningful work focused on improving patient care and outcomes.
