Key Responsibilities

• Conduct thorough psychosocial assessments to evaluate patients' mental health status, identifying strengths and areas for support.
• Deliver evidence-based therapeutic interventions, including cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), tailored to individual patient needs.
• Collaborate with a multidisciplinary team to ensure coordinated care and effective treatment strategies.
• Educate patients and their families about diagnoses, treatment options, and managing medications to empower informed decision-making.
• Facilitate individual, group, and family therapy sessions, utilizing proven methodologies to foster healing and understanding.
• Develop and implement comprehensive treatment plans in collaboration with the healthcare team, ensuring alignment with patients' goals and preferences.
• Assist patients in navigating the often complex mental health system, advocating for their rights and ensuring their needs are prioritized.
• Lead psychoeducation sessions and therapeutic groups, enhancing understanding of mental health issues among patients and their families.

Patient Population / Client Focus

This position primarily serves patients within a hospice setting, where emotional support is paramount. Qualifications

• A Master’s degree in Social Work from an accredited program is required.
• A current and active state licensure as a Licensed Clinical Social Worker (LCSW).
