Key Responsibilities
- Conduct thorough psychosocial assessments to evaluate patients' mental health status, identifying strengths and areas for support.
- Deliver evidence-based therapeutic interventions, including cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), tailored to individual patient needs.
- Collaborate with a multidisciplinary team to ensure coordinated care and effective treatment strategies.
- Educate patients and their families about diagnoses, treatment options, and managing medications to empower informed decision-making.
- Facilitate individual, group, and family therapy sessions, utilizing proven methodologies to foster healing and understanding.
- Develop and implement comprehensive treatment plans in collaboration with the healthcare team, ensuring alignment with patients' goals and preferences.
- Assist patients in navigating the often complex mental health system, advocating for their rights and ensuring their needs are prioritized.
- Lead psychoeducation sessions and therapeutic groups, enhancing understanding of mental health issues among patients and their families.
Patient Population / Client Focus
This position primarily serves patients within a hospice setting, where emotional support is paramount. Qualifications
- A Master’s degree in Social Work from an accredited program is required.
- A current and active state licensure as a Licensed Clinical Social Worker (LCSW).
- In-depth knowledge of mental health disorders, treatment modalities, and crisis intervention techniques.
- Ability to foster collaborative relationships within a multidisciplinary team to enhance patient care.
- Strong interpersonal, communication, and organizational skills.
Preferred Experience
- Previous experience working in hospice or palliative care settings is highly desirable.
