Will be in ASC outpatient clinic but will be required to float to Main OR as needed. Scrub is required as they will circulate and scrub. Start date: ASAP. Ratios: 1:1. Years of experience REQ: 5. First-timers accepted: Yes. Weekend REQ: None. Certs REQ: ACLS, BLS. Is on-call REQ?: No. Will this traveler need to float between like units? Yes - Main OR. Open to accommodating block schedule? Per manager discretion. Pending License accepted: No. RTO Restrictions: RTO must be included at time of submission. Guaranteed Hours: Facility is able to call off once per two week pay period.

Patient Diagnoses: Our top service lines are: gyn, ent, and uro. We also perform about 5 davinci robotic cases per week and about 3 Ortho cases. Robotic experiences for surgical techs is preferred. Scrub experience is required for circulators.

• Must include response to the following from the candidate with submission: Have you ever worked at a Providence facility as a permanent employee, on a travel contract, or as registry/per-diem? If yes, please include location and dates worked.
• LA fire card within the last 30 days is required (Aya/Facility will not reimburse for this. Recruiters can margin for it accordingly).
• References Required: 2 References covering 1 year (6 months for CNAs) within the last 3 years during the compliance process and prior to start. Can include Charge RNs/Lead
• ALL travelers are expected to float between all facilities within a given state. travelers' refusal to float may result in termination of assignment and/or forfeiture of guaranteed hours.
• If required for the assignment, Client agrees to supply Providers with communication devices (i.e. cell phone, pager, etc.) needed to perform the duties as assigned at no cost to Provider or Agency. If Client does not provide a mobile phone required for the assignment, Client agrees to pay Agency monthly, for all work-related cell phone use by each Provider, up to a maximum amount of $50.00 as invoiced by Agency. Providers will be responsible for submitting monthly cell phone bill directly to Agency.
• Modules: Modules are non-billable, average 16 hours spent. Over 20 hours requires PM approval. Modules are completed pre-start, and annually. Time spent on modules is self-reported, completion is recorded in Workramp transcripts.
