Key Responsibilities\n

As a Licensed Mental Health Counselor, you will engage in a variety of essential duties, including:

  • Maintaining and preparing electronic medical records in compliance with our health system’s policies while ensuring timely written and oral reports for patient hand-offs.

  • Conducting thorough psychosocial assessments for patients and families during admission, annually, or as clinically indicated to identify social, emotional, and physical needs, applying relevant social work theories and techniques.
  • \n
  • Collaborating effectively with interdisciplinary healthcare teams to develop and implement individualized psychosocial treatment plans and coordinating referrals to specialty care and social services.
  • \n
  • Supporting patients and families in addressing environmental and psychological challenges associated with illness, ensuring they have the resources they need to cope.
  • \n
  • Managing an independent caseload with varying levels of clinical acuity, guaranteeing appropriate attention to each patient’s unique circumstances.

  • Conducting proactive case finding for clients at risk of psychosocial issues and participating in team conferences and interdisciplinary rounds for enhanced care delivery.

  • Providing essential therapeutic services including supportive counseling, grief counseling, and crisis intervention to help patients navigate illness and related life changes.

  • Offering psychoeducation to improve understanding of diagnoses, treatments, and prognoses; facilitating transitions from youth to adult services for patients and their families.

  • Engaging in quality assurance initiatives to enhance social work services and contributing to policy reviews and updates related to mental health practices.

  • Making mental health diagnoses as part of comprehensive treatment planning and providing clinical supervision to MSWs and student interns to cultivate their professional development.
