This travel role focuses on providing home health physical therapy services, ensuring patients achieve maximum independence and function. Key Responsibilities of the Physical Therapist

Perform physical therapy evaluation and treatment based on physician orders/referrals
Complete comprehensive evaluations using appropriate tools and procedures
Review patient records for current diagnosis, history, precautions, and contraindications
Establish measurable goals and develop treatment plans for patient independence
Regularly reassess treatment effectiveness and patient progress
Communicate patient needs and progress to treatment team and family
Consult with medical providers on patient evaluation, treatment, and discharge
Assist in securing and incorporating special adaptive equipment
Maintain and submit proper documentation as per regulations
Supervise Physical Therapist Assistants and Aides in patient care
Attend and participate in staff development programs

Requirements of the Physical Therapist

KY Physical Therapist Licensure or Compact Licensure
1 year home health physical therapy experience
Previous oasis experience
Experience with Home Care Home Base
