Work Environment

This position is onsite, providing care directly to patients in their homes. The work setting fosters a collaborative atmosphere where you will work alongside a dynamic team of healthcare professionals including nurses, occupational therapists, and speech-language pathologists. You will have the autonomy to drive patient treatment decisions while being supported by administrative and operational staff to facilitate seamless care delivery.

Key Responsibilities

• Conduct thorough evaluations of patients, utilizing a range of assessment tools to identify impairments, functional limitations, and potential health status changes resulting from injuries or illnesses.

• Develop and implement individualized treatment plans based on physicians' orders, incorporating evidence-based practices to achieve measurable goals for each patient.

• Maintain comprehensive and accurate documentation of patient care, including evaluations, treatment plans, progress notes, and discharge summaries in a timely manner.

• Supervise and mentor support staff, including Physical Therapist Assistants (PTAs), ensuring high-quality patient care and adherence to established protocols.

• Engage in communication with interdisciplinary teams, including case managers and physicians, to assess and modify treatment plans based on patient progress and changing needs.

• Provide constructive feedback to management, contributing to departmental goals and a positive workplace culture that encourages professional growth and development.

Patient Population / Client Focus

The patients you will serve include individuals recovering from surgery, managing chronic conditions, or experiencing mobility challenges. Your role is pivotal in helping patients regain independence and improve their quality of life through targeted physical therapy interventions. Qualifications

• A Bachelor of Science degree in Physical Therapy from an accredited university.

• Current licensure as a Physical Therapist in the state of Virginia.

• Strong written and verbal communication skills, with the ability to engage effectively with diverse populations.

• Proficient in using electronic health record (EHR) systems and basic computer skills.

Preferred Experience

• A minimum of two years of experience working as a Physical Therapist, ideally within a home care or outpatient setting.

• Experience supervising or training support staff, including PTAs.

• Demonstrated ability to work collaboratively in a multidisciplinary team environment.
