As a Home Health Physical Therapist, you play an essential role in the care team, assisting patients in their recovery, enhancing mobility, and preserving independence in the environment where they feel most secure: their home.

This position offers a high degree of autonomy and is centered around building relationships, allowing you to witness the tangible effects of your efforts daily.

Your Responsibilities

• Deliver Patient-Centered Care
  • Assess and evaluate patients’ functional abilities, mobility, strength, and rehabilitation potential.
  • Collaborate with physicians and the care team to create and implement personalized care plans.
  • Administer hands-on therapy to enhance mobility, strength, balance, and independence.
  • Educate patients and their caregivers on treatment protocols, exercises, and safety measures.
• Drive Outcomes & Recovery
  • Track patient progress and modify treatment plans as necessary.
  • Facilitate safe transitions of care and assist in discharge planning.
  • Identify additional patient needs and work with interdisciplinary teams.
• Collaborate & Communicate
  • Work closely with nurses, therapists, and physicians to ensure cohesive care.
  • Accurately document patient care and progress in the electronic medical record (EMR).
  • Engage in case conferences and team meetings.
• Lead & Support
  • Oversee Physical Therapist Assistants (PTAs) in line with state regulations.
  • Provide mentorship and education to caregivers and support staff.

Qualifications and Skills

• Graduation from an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state where you practice.
• 1–2 years of experience preferred, particularly in home health.
• Strong clinical assessment and critical thinking abilities.
• Excellent communication skills and patient education capabilities.
• Current CPR certification.
• Valid driver’s license, dependable transportation, and auto insurance.
