Belle is seeking a compassionate Nurse Practitioner to provide remote telehealth support for Medicare beneficiaries enrolled in the Chronic Care Management program. This role focuses on urgent non-emergent clinical escalations and requires collaboration with nurses and care teams. You will assess patients remotely, recommend evidence-based interventions, and guide escalation to PCP, ED, or 911 when appropriate, helping keep members healthy and out of the hospital. #J-18808-Ljbffr Belle
