POSITION PURPOSE

This position reports directly to the Clinical Director of Alpha Recovery Center. With supervision, the Case Manager is responsible for providing guidance to clients to help them achieve their highest level of independence. Case management is a collaborative process which assesses, plans, implements, coordinates, monitors, and evaluates the selection of services to optimally meet each individual clients’ needs. The Case Manager coordinates and monitors clients’ clinical, medical, and psychosocial needs throughout treatment. The role ensures continuity of care and compliance with DHCS, Title 9/22, HIPAA, 42 CFR Part 2, and Joint Commission standards.

 

POSITION REQUIREMENTS/QUALIFICATIONS

EDUCATION:

High School Graduate or equivalent. Associate’s or Bachelor’s degree in related field. 

EXPERIENCE:

One year of experience in social services or related field.

Experience working with diverse populations of men and women Certification in Substance Abuse Counseling preferred.

Experience facilitating support groups preferred.

Strong oral and written communication skills

LICENSURE/CERTIFICATION:

Valid California Driver’s License approved by Alpha Recovery Center’s Insurance Carrier. Current Registration or Certification by an approved DHCS Certifying Organization

Demonstrates Proficiency in Verbal & Written Communication Skills.

Knowledge of State & Federal Statutes Regarding Patient Confidentiality Laws.

Knowledge of Drug-Free Workplace Policies.

Knowledge of Workplace Violence.

Knowledge of Corporate Integrity & Compliance Program.

Knowledge of current utilized technology.

Knowledge of 12-Step Programs (addiction and recovery)

Position is required to work indoors and outdoor environment as needed.

Potential exposure to violent situations.

Potential exposure to airborne/blood-borne pathogens or other potentially infectious materials.

Telephone System, Computer, Copy Machine/Fax/Scanner/ Automobile / Van / Bus

PHYSICAL & MENTAL DEMANDS:

May include operating motor vehicles, lifting up to 50 lbs., constant monitoring of clients and crisis intervention.

PERSONAL PROTECTIVE EQUIPMENT:

Safety Belt Safety Belt Safety Belt, gloves, masks, goggles, gowns.

Slip and Falls, violence, potential exposure to biohazard waste.

Performance standards are integrated within the position responsibilities. The principle functions of the position identified shall not be considered as a complete description of the all the work requirements and expectations that may be inherent in the position.

SPECIFIC AREAS OF RESPONSIBILITY TO POSITION

1. Facilitates client intake process and adds to the client EMR file for the program and teammates, ensuring all Case Management documentation is comprehensive and complete.

2. With supervision, collaborates with clients to complete Intake Assessment.

3. Assists all Case Managers in maintaining confidential client files in a professional and timely manner ensuring all forms are the most recently revised versions and in compliance with organization standards; all required documents are present, complete, signed by the client and staff as applicable; and documents are placed in the correct EMR.

4. Meets with clients as specified by program requirements and documents progress in required documents.

5. Builds positive relationships with clients and staff.

6. Educates clients on rules and responsibilities.

7. Prepares notes for Treatment Team as directed to ensure comprehensive communication and LOC coordination

8. Conducts chemical dependency group and individual counseling sessions.

9. Contributes to discharge summaries that are approved and signed by Program Director, or Clinical Director.

10. Coordinates discharge and post-discharge planning

11. Assists in quality assurance activities, data collection, and preparing reports

12. With supervision, facilitates client life skills classes

13. Assists in providing specialized case management services and life skills groups/ classes to client’s as applicable.

14. Maintains confidentiality in compliance with the Health Insurance Portability and Accountability Act (HIPAA) standards.

15. Attends scheduled team meetings and trainings.

16. Assists with maintaining compliance with Alpha Recovery Center’s regulations and organization protocols.

17. Communicate in a professional manner with proper use of telephone etiquette.

18. Perform in a professional manner and role model a positive image of mental health.

19. Knowledge of policies and procedures pertaining to attendance and punctuality.

20. Reports to work as assigned.

21. Understand and adhere to legal guidelines pertaining to client confidentiality and client rights.

22. Maintains acceptable overall attendance.

23. Perform duties independently, taking a proactive approach to problem situations, seeking supervision on an as needed basis.

24. Seeks out learning experiences and incorporates new knowledge into practice.

25. Reports incidents, accidents, and occurrences in accordance with policies and procedures.

26. Maintains safety of the physical environment.

27. Perform other tasks and duties as assigned.
