We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our team and provide high-quality, patient-centered care in a supportive outpatient environment.

RESPONSIBILITIES:

' Provide comprehensive outpatient primary care to adult patients.
' Evaluate, diagnose, and treat acute and chronic medical conditions.
' Perform routine physicals, preventive health screenings, and wellness visits.
' Manage chronic conditions including hypertension, diabetes, hyperlipidemia, obesity, asthma, and cardiovascular disease.
' Develop individualized treatment plans and provide medication management.
' Order and interpret laboratory tests, diagnostic imaging, and other appropriate studies.
' Provide patient education regarding disease prevention, medications, nutrition, exercise, and healthy lifestyle choices.
' Coordinate referrals and care with specialists and other healthcare professionals.
' Maintain accurate and timely documentation in the electronic health record.
' Participate in quality improvement and patient safety initiatives.
' Build strong, long-term relationships with patients and their families.

QUALIFICATIONS:

' MD or DO degree from an accredited medical school.
' Completion of an accredited residency in Family Medicine, Internal Medicine, or another applicable primary care specialty.
' Current, unrestricted medical license.
' Board certified or board eligible in the appropriate specialty.
' DEA registration or eligibility to obtain one.
' Current BLS certification.
' Excellent clinical judgment and communication skills.
' Strong commitment to compassionate, patient-centered care.
' Ability to work effectively as part of a collaborative healthcare team.
