- Manage a caseload of patients with chronic conditions by assessing needs and creating individualized care plans.
- Coordinate care across providers, clinics and community programs to ensure patients receive timely services and care.
- Provide education and coaching to patients and families to promote understanding, self-management, and treatment adherence.
- Collects and maintains case management data, including service utilization, identified barriers, interventions, and patient outcomes.
- Use patient data to identify care gaps, improve coordination processes, and support quality improvement efforts.
