Summary: Following the South County Fire (SCF) core values, this position will support direct case management and perform intake and referral processes as part of our Community Resources Paramedic (CRP) Program. South County Fire seeks to connect community members in need with appropriate local resources. This role will bridge gaps in care for patients who historically fall through the cracks and are not connected with necessary services. As part of the work schedule, this position will spend a portion of the workweek working directly with the Lynnwood Police Department to support coordinated response efforts and interdepartmental collaboration.

Essential Job Functions

Essential Job Duties and Responsibilities:
  • Build strong relationships with local and regional social services that can benefit the community members of the Regional Fire Authority (RFA).
  • Document and track all community member contact and progress in accordance with SCF documenting systems.
  • Identify resources and services for community members in need including mental/behavioral health, substance use, homelessness/housing, financial, and other social service needs.
  • Connect community members to appropriate resources and ensure follow through.
  • Assist and advocate for community members with scheduling appointments, arranging transportation, complete applications for programs for which they may be eligible, etc.
  • Conduct intakes to assess the needs of community members to determine the appropriate type of community and/or clinical support.
  • Collaborate with providers and systems to reduce barriers and inequities related to the health and cultural needs of the individual and community/population being served.
  • Provide appropriate health education and information to support the community members' engagement in healthy behaviors and lifestyle choices.
  • Facilitate secure transition of care by ensuring needed services and supports are in place.
  • Demonstrate respect for diversity and a commitment to developing cultural humility.
  • Develop and document individualized response plans for community member and households.
  • Maintains printed and electronic files, and correspondence adhering to HIPPA laws.
  • Perform other work-related duties as assigned.
Required Knowledge, Skills and Abilities

Knowledge Of:
  • HIPAA compliance as related to the continuation of care.
  • Knowledge of basic medical terminology.
  • Social services available within the community, greater Snohomish County, and the State of Washington, as well as any federally related programs.
Skills and Ability To:
  • Communicate professionally with others both verbally, written, and electronically.
  • Use technical and non-technical language to include but not limited to, firefighters, police officers, and medical professionals.
  • Understand and follow verbal and/or written policies, processes, and instructions.
  • Exercise independent judgment and initiative in managing multiple projects and assignments.
  • Establishing and maintaining effective working relationships with both external and internal customers.
  • Willing to travel to multiple office locations and community members' homes throughout the RFA.
  • Become familiar with, follow, and actively support South County Fire RFA's mission, vision, and core value statements.
  • Utilize MS Office suites and Google Suite.
Education and Experience:
  • High school diploma or GED required
  • Previous experience in community health services, education, motivational interviewing, connecting community members to assistance and/or mental health first aid for first responders desired.
  • Certified peer counselor or lived experience with behavioral health, substance use or system involvement preferred
Additional Requirements

Special Requirements :
  • Must have and maintain a valid WA State Driver's License with acceptable driving record.
  • Must pass a formal criminal background check as required by RCW 43.43.830. as well as a formal background check through Lynwood Police Department.
  • Willing to work some weekends on a rotating basis with team to ensure coverage over entire week.

Physical Demands:
The employee is regularly required to stand, walk, use hands to manipulate, handle, or operate objects, controls, or tools as listed above, reach with hands and arms, climb, balance, stoop, kneel, or crouch. Must be able to assist patients and lift when necessary, utilizing lift assistance devices. Office activities require the ability to sit for long periods of time and move throughout the office or field; ability to perform manipulative skills required for word processing; ability to lift, carry, and maneuver various educational supplies and equipment up to 50 pounds; ability to read computer screen, various documents and forms; hearing and speech abilities to effectively communicate and perform duties.


Working Conditions:
Work takes place in a variety of environments including office settings, classrooms, meetings, and public events which may take place outdoors in inclement weather. Travel may involve visiting challenging and uncomfortable living situations. CHW employees work on a rotating schedule to ensure weekend coverage.
