Community Outreach Specialist

Conduct high-volume outreach to engage patients in preventive care and chronic disease management programs. Build trusted relationships through compassionate, patient-centered communication and education. Help patients overcome barriers to care, including transportation, scheduling, language, health literacy, and financial challenges. Support patients in completing recommended screenings and follow-up care, including breast and colorectal cancer screenings. Provide education and follow-up support for patients managing chronic conditions such as hypertension and diabetes. Connect patients with community resources and coordinate referrals, transportation, and other non-clinical support services. Accurately document patient interactions, outreach efforts, referrals, and outcomes in designated systems. Collaborate with care teams and community partners to improve patient engagement, access to care, and health outcomes. Maintain compliance with HIPAA, organizational policies, quality standards, and outreach protocols. Requirements High school diploma or GED required; associate degree or relevant certification preferred. Basic understanding of medical terminology Experience working with diverse, underserved, or community-based populations. Experience conducting in-home visits and community outreach is preferred. Valid driver’s license and reliable transportation, as required for home visits. Basic proficiency with technology, including EHRs, mobile documentation tools, or care management platforms. Bilingual or multilingual skills reflective of the communities served strongly preferred. Prior experience in community health, outreach, care coordination, or public health programs. Colorado Community Health Worker certification through the Colorado Department of Public Health and Environment (CDPHE) is preferred, or ability to obtain certification within a defined timeframe, as required by state or program standards.
