Population Health Nurse: RPM & Care Coordination Leader

Well Care Community Health, Inc. is seeking a Population Health Nurse to serve as a clinical lead for the chronic-disease hub. You will translate EHR, registries, and RPM data into outreach and intervention while coordinating with Case Manager, CHW, providers, and pharmacy.
