Locations North Austin Round Rock Pflugerville Giddings Surrounding Areas Position Overview The Licensed Practical Nurse (LPN/LVN) is responsible for providing and documenting skilled nursing care under the supervision of a Registered Nurse, following the developed care plan and physician orders while complying with confidentiality standards and professional boundaries. Essential Job Functions Delivery and coordination of quality patient care in compliance with physician orders. Continuous observation and assessment of patient condition, reporting changes to supervisor/physician. Timely documentation of all activities, assessments, nursing actions, responses, and care coordination. Participation in, implementation of, and updating the nursing care plan. Executing nursing actions based on assessment to achieve expected outcomes. Recognizing changes in patient needs and implementing interventions to prevent or reduce risk. Compliance with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act. Providing infection control measures in accordance with OSHA standards. Educating patients and families on disease process, self‑care, and prevention strategies. Maintaining competency through educational programs, continuing education units, and skill evaluations. Requirements Graduate of an accredited nursing school. Current, unrestricted state license as an LPN/LVN. Current CPR certification (hands‑on). Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care policies. Reliable transportation. State‑specific requirements: South Carolina – 1 year of pediatrics experience. California – 1 year of experience under current nursing license. Louisiana – 1 year of experience as a licensed nurse. Additional preferred experience of 6 months recent nursing in a clinical setting or home health experience. Travel to patient homes. Lift up to 50 pounds. Reposition patients and move equipment without assistance. Prolonged walking, standing, bending, kneeling, reaching, twisting. Ability to sit and climb stairs. Visual and hearing acuity. Strong sense of smell and touch. Respond physically and mentally to emergencies in the home or during transport.
