Salary Range: $43,901.00 - $69,639.00 Annually

Hiring Salary Range: $43,901.00 - $56,770.00 Annually

Job Summary: A Grant Services Specialist II provides a wide variety of grant-funded paraprofessional administrative, programmatic and complex general support services work to an assigned City agency or program.

Work of this class involves no supervisory duties or responsibilities. Incumbents receive general supervision from an administrative superior. Employees in this class work a conventional workweek that may include evening and weekend hours. Work is generally performed in an office or clinic where working conditions are normal. Work generally requires minimal physical exertion.

The Dementia Navigation Program Assistant will provide administrative, programmatic, and community outreach support for the Dementia Navigation Program. The role is responsible for assisting with the coordination of educational programs, outreach events, and community engagement activities while serving as one point of contact for individuals living with dementia, their caregivers, and community partners. The Dementia Navigation Program Assistant helps connect callers to appropriate resources and supports the successful delivery of dementia- related services and initiatives. The Dementia Navigation Program Assistant will be crossed trained to assist the Maryland Access Point Call Center when needed.

Essential Functions

• Provide administrative and operational support for the dementia navigation program.
• Answer phone calls, emails, and inquiries from individuals, caregivers, and community partners in a professional, compassionate, and timely manner.
• Connect individuals and families with appropriate community resources, services, and referrals based on their needs.
• Coordinate event registrations, confirmations, and follow-up communications.
• Support outreach efforts by communicating with community organizations, healthcare providers, and partner agencies.
• Collect and organize program data for reporting and evaluation purposes.
• Ensure confidentiality of participant information and comply with organizational policies and regulations.
• Perform other related duties as assigned to support the dementia navigation program.
• Assist with Maryland access point call center when needed

Minimum Qualifications

• Education: Have a bachelor's degree from an accredited college or university.
• AND
• Experience: One year of experience in coordinating, monitoring and processing cases involving the provision of health, social, vocational or rehabilitation services to clients or equivalent combination of education and experience.
• OR
• Equivalency Notes: Have an equivalent combination of education and experience.

Preferred Qualifications:

• Education: Associate degree in human services, social work, public health, healthcare administration, or a related field preferred; equivalent experience and/or education will be considered.
• Experience: Two years of experience in administrative support, community outreach, healthcare, aging services, call center experience, or a related field preferred

• Knowledge of the techniques of preparing data and statistical reports and developing forms for data and statistical studies.

• Skill in motivating others to follow a designated regimen or course of action to effect behavior modifications.
• Skill in coordinating responses from multiple public and private agencies, institutions and community resource organizations.

• Skill (where required) in speaking, reading and writing a specified non-English language and interacting within associated cultural idioms, customs and norms.
• Ability to establish and maintain effective liaisons with people of varied backgrounds, including clients, families, neighbors, government representatives and public and private community institutions, service providers and personnel.
• Ability to promptly deal with emergency or crisis situations with calm, tact and dispatch.
• Ability to deal courteously with other employees and the public, both in person and by telephone.
• Ability to exercise sound judgment on a variety of complex matters guided by general business, professional or standard administrative instructions, processes and procedures.

• Experience working with older adults, caregivers, or individuals living with dementia is preferred.
• Bilingual skills are a plus.

Additional Information

Background Check Eligible candidates under final consideration for appointment to positions identified as positions of trust will be required to complete authorization for a Criminal Background Check and/or Fingerprint screening, and must be successfully completed.

Probation All persons, including current City employees, selected for this position must complete a 6-MONTH mandatory probation.
