Key Responsibilities

• Engage clients in developing goal-oriented plans addressing physical, emotional, social, and environmental needs.

• Link clients to community and formal resources while promoting independence and self-sufficiency.

• Coordinate care among multiple service providers and support clients through recovery and stabilization.

• Assist individuals with mental health and/or substance use diagnoses who have experienced chronic homelessness in obtaining permanent housing with no readiness requirements.

• Provide outreach, advocacy, and service coordination in varied environments (streets, apartments, shelters, and offices).

• Utilize a trauma-informed, harm-reduction, and Housing First approach in all interactions.

• Assess Social Determinants of Health (SDOH), housing history, and health risk factors.

• Collaborate with nursing and clinical staff to determine appropriate follow-up assessments and referrals.

• Develop and implement individualized service plans and monitor progress toward goals.

• Coordinate services between systems, facilitate access to resources, and advocate for clients when necessary.

• Maintain accurate, timely, and professional documentation in accordance with agency standards.

• Build and maintain positive working relationships with colleagues and community service providers.

• Participate in Housing First trainings, team meetings, and consultations with the County.

• Pursue ongoing professional development and supervision to enhance cultural competence and service quality.

• Adhere to confidentiality standards and agency policies.

Qualifications

• Bachelor’s degree in Social Work, Human Services, or related field, or equivalent combination of education and experience.
• Valid driver’s license required.
• Experience in case management, shelter services, or working with individuals experiencing homelessness preferred.
• Computer proficiency: HMIS, Credible, ADP, Microsoft Office Suite.
• Strong skills in communication, crisis intervention, problem-solving, and culturally responsive care.
