PT/OT/SLP/PTA/COTA 1099 - NEW GRADS WELCOME

$50 - $62 per hour

Rehabilitation & Care Center

Job Description

Physical Therapist (PT), Occupational Therapist (OT), Speech Language Pathologist (SLP), Physical Therapist Assistant (PTA), Certified Occupational Therapy Assistant (COTA) needed for Full-time, Part-time, Weekend Contractor (1099) at a Skilled Nursing Facility in Bronx (10469) and Queens (Whitestone area 11357).

Candidate must have a valid NY license, has EMR experience (NetHealth, Sigma or PCC), be able to provide skilled rehabilitation services in a Skilled Nursing Facility (SNF) setting.

General duties and responsibilities:

Provide appropriate and timely evaluation of patients’ rehabilitative needs (PT, OT, SLP)
Develop therapy care plans using a variety of treatment techniques to address functional impairment and deficits
Provide quality, personalized and evidence-based care, and proven interventions
Motivate patients during treatment to help them participate and function optimally
Monitor, re-assess effectiveness, and communicate progress and potential barriers to therapy
Provide timely and appropriate documentation of skilled care provided

Job Requirements:

Valid NY State License
CPR Certification (Preferred)
Medical/Physical
Good organization and time-management skills
Good written skills
Good interpersonal skills

Job Type:

Full-time contractor (1099)
Part-time Contractor (1099)
Weekend Contractor (1099)

Pay:

PT/OT/SLP rate range: $50.00 - $62 per hour
PTA/COTA rate range: $30 - $42per hour

*** All rate ranges offered are based on position and years of applicable experience
