Hospice LPN

Pay rate: $36/hr

Location: Salisbury, MD

Schedule: 8a-4:30p (M-F), 40 hours per week

Contract length: 8 weeks

Coverage areas:

• Dorchester County
• Wicomico/Worcester Counties

Key Responsibilities:

• Deliver hospice patient care while maintaining accountability for clinical outcomes
• Manage end-of-life care in patients' homes
• Conduct initial evaluations within 48 hours of referral, initiate personalized plans, and collaborate daily with physicians and care teams
• Provide critical symptom management, psychosocial support, and ensure meticulous clinical documentation
• Exercise sound clinical judgment in an effective and efficient manner
• Coordinate patient care with interdisciplinary team members and caregivers
• Provide comfort-focused and end-of-life care to patients and families
• Demonstrate critical thinking and problem-solving skills in patient management
• Maintain compliance with established nursing standards and best practices
• Assist other healthcare professionals in the management and coordination of patient care
• Accurately complete documentation, including OASIS assessments when required
• Support patients and families through education, advocacy, and emotional support

Assignment Details:

• Admissions and OASIS assessments
• LPN wound care visits as needed
• Home-based hospice patient care throughout assigned coverage area
• Documentation completed within OASIS charting system

Requirements:

• Active LPN license in the state of practice
• Minimum 1 year of home health experience
• OASIS experience preferred
