Required Skills & Experience
• \tActive Occupational Therapist (OT) license in good standing.\n•\tGraduate of an accredited Occupational Therapy program.\n•\tMinimum of 1 year of Occupational Therapy experience.\n•\tPrevious home health experience preferred.\n•\tCurrent CPR/BLS certification.\n•\tStrong knowledge of rehabilitation techniques and adaptive equipment.\n•\tExcellent assessment, communication, and organizational skills.\n•\tAbility to work independently in the home care setting.

Job Description

We are seeking a compassionate and experienced Occupational Therapist (OT) to provide skilled therapy services to patients in their homes. This role is ideal for a clinician who enjoys working independently while helping patients regain functional independence, improve quality of life, and remain safely in their home environment.\nThe Occupational Therapist will evaluate patients, develop individualized treatment plans, and provide therapeutic interventions focused on activities of daily living (ADLs), mobility, safety, and overall functional performance. The ideal candidate will have prior home health experience and be comfortable traveling to patient homes within a 25-mile service area. \n\nResponsibilities\n•\tConduct comprehensive occupational therapy evaluations in patients' homes.\n•\tDevelop and implement individualized treatment plans based on patient needs and goals.\n•\tProvide therapy focused on improving activities of daily living (ADLs), instrumental activities of daily living (IADLs), strength, coordination, and functional mobility.\n•\tAssess home environments and recommend modifications, adaptive equipment, and safety interventions.\n•\tEducate patients and caregivers on therapeutic exercises, fall prevention, energy conservation, and adaptive techniques.\n•\tMonitor patient progress and modify treatment plans as needed.\n•\tCollaborate with physicians, nurses, physical therapists, speech therapists, and other members of the healthcare team.\n•\tComplete accurate and timely clinical documentation in accordance with agency standards and regulatory requirements.\n•\tMaintain compliance with all Medicare, state, federal, and agency guidelines.\n•\tTravel to patient homes within a 25-mile radius of the 48076 zip code.
