Work Environment

This role is primarily onsite at various affiliated assisted living facilities throughout the picturesque Elmwood Park community. You will enjoy a Monday to Friday work schedule, allowing a healthy work-life balance with hours from 9 AM to 5 PM. Each day will offer a blend of autonomous work within a supportive team structure, making it a fulfilling role for those dedicated to patient-centered therapy.

Key Responsibilities

• Deliver high-quality on-site therapy services directly to residents within assisted living facilities, ensuring respectful and individualized care.
• Conduct thorough evaluations of residents to develop customized, orthopedic-informed treatment plans targeting their specific needs and conditions.
• Engage in effective communication and collaboration with facility nursing staff, caregivers, and our internal team of medical professionals to create comprehensive care strategies.
• Educate both residents and their families on safe mobility practices, effective home exercise programs, and overall wellness strategies to promote independence.
• Utilize mobile electronic medical records (EMR) systems for efficient, HIPAA-compliant documentation of patient progress and treatment outcomes.
• Serve as an essential liaison between the assisted living facilities and our multidisciplinary clinic, facilitating referrals and follow-up care plans.

Patient Population / Client Focus

The primary focus of this role is to serve the geriatric population residing in assisted living facilities. You will be working with older adults who may present with various orthopedic conditions, mobility challenges, and other health concerns common in aging populations. Your expertise will play a crucial role in enhancing their daily living and overall health.

Qualifications

• A valid Physical Therapist license in the State of New Jersey or eligibility for licensure.
• A minimum of two years of experience in outpatient therapy, geriatrics, or similar environments such as assisted living facilities, skilled nursing facilities, or home health care.
• Exceptional interpersonal and communication skills, vital for effectively connecting with patients, families, and the healthcare team.
• Clinical knowledge and hands-on experience dealing with common orthopedic conditions affecting older adults, allowing for comprehensive treatment and care.
• Demonstrated ability to work independently and engage effectively within a multidisciplinary team in residential care settings.

Preferred Experience

• Experience in developing and implementing therapeutic exercise programs tailored to geriatrics.
• Proficiency using mobile EMR and other digital tools to streamline patient documentation processes.
• A background in home health care where you collaborated with multifaceted healthcare teams.

Compensation and Benefits
