An exciting opportunity is available for a Full-Time Physical Therapist (PT) to join our team at a Skilled Nursing Facility (SNF) in Monticello, Minnesota . Key Responsibilities:

• Patient Assessments & Treatments : Assess and treat patients based on individualized care plans to meet their unique needs.
• Documentation : Accurately document patient progress, treatments, and outcomes in a timely manner.
• Program Development : Plan, market, and develop new programs to enhance patient care and improve overall outcomes.
• Effective Communication : Collaborate with patients, families, physicians, and healthcare professionals to ensure coordinated care.
• Staff Education : Lead in-service training sessions for fellow therapists and staff.
• Clinic Operations : Assist with the general operations of the therapy department, maintaining high standards of care and efficiency.

Qualifications:

• Licensure : Current Physical Therapist (PT) licensure in Minnesota (or eligibility to obtain licensure).
