Lead the formation and implementation of Child and Family Teams (CFT), ensuring the team includes family members, county representatives, the client, and other selected participants. Act as liaison with external agencies and convene CFT meetings. Provide case management services aimed at stabilizing clients and reducing involuntary hospitalization or placement disruption. Offer mental health services such as individual rehabilitation, collateral intervention, and coaching when indicated. Ensure all team members are active participants in the CFT process, helping to generate a mission statement and prioritize needs. Collaborate with team members to adhere to program process and principles, aligning with TEAMMATES philosophy of "Whatever It Takes." Qualifications Education - Facilitator II: Master’s degree in Mental Health, Human Services, or closely related field. Education - Facilitator I: Bachelor’s degree in Mental Health, Human Services, or closely related field. Experience - Facilitator II: One year of experience in a mental health field and direct service experience with severely emotionally disturbed children, adolescents, families, and high‑risk families. Experience - Facilitator I: Two years of experience in a mental health field and direct service experience with severely emotionally disturbed children, adolescents, families, and high‑risk families.
