Licensed Vocational Field Nurse (LVN)Desire Home Care, Inc. is seeking a compassionate and experienced Licensed Vocational Field Nurse (LVN) to join our team as a shift healthcare employee. This individual contributor role will be based in the Riverside, Fontana, and Murrieta areas and will involve providing healthcare services to patients with intellectual and/or developmental disabilities in the comfort of patients' homes. As an LVN Home Health Shift Nurse, you will be responsible for providing treatment, medication administration, and supervision according to their specific health needs, while also adhering to all regulatory and ethical standards.Compensation- Shift Care:Routine Follow-Up Visit: $25-30 per hourBenefits:Daily pay: Access your earned wages the same day you work!Medical, dental, vision401K with 4% matchPaid time offPaid sick timeReferral payPaid trainingsFirst time home buyer program assistanceSign on bonus: Complete 100 visits within 90 days & receive a $500 bonusResponsibilities:Provide skilled nursing care to patients in their homes, in accordance with established care plans and physician's ordersMonitor and assess patients' health status and report any changes or concerns to the supervising Registered NurseAdminister medications and treatments as prescribed by the physicianProvide wound care, catheter care, G-tube care, and other nursing procedures as neededEducate patients and their families on their conditions, medications, and self-care techniquesCollaborate with other healthcare professionals to coordinate and manage patient careMaintain accurate and up-to-date patient records, including progress notes, medication administration records, and nursing assessmentsAdhere to all regulatory and ethical standards, including HIPAA guidelines and infection control protocolsCommunicate effectively with patients, families, and other team members to ensure the best possible outcomes for patientsRequirements: *New Graduates Welcomed*Active and unrestricted LVN license in the state of CaliforniaMinimum of 1 year of experience as an LVN, preferably in a home health settingHave experience working with patients diagnosed with autism, cerebral palsy, down syndrome, and/or epilepsyHave experience working in pediatricsStrong knowledge of nursing principles, practices, and proceduresExcellent clinical assessment and critical thinking skillsAbility to work independently and manage time effectivelyStrong communication, interpersonal, and organizational skillsMust have a valid driver's license and reliable transportationMust have current BLS certificatePhysical and TB clearance valid within the yearAbility to complete documentation via agency EMR systemSHIFTS:Minimum of 6 hour shifts, no more than 8 hours, no holiday shiftsMorning, Afternoon, and EveningTo achieve a full 8-hour workday, you may be assigned multiple patients, and shifts typically vary in lengthEEOC Statement: Desire Home Care, Inc. is an equal opportunity employer and is committed to creating a diverse and inclusive workplace.
