- Conduct evaluations for patients in both inpatient and outpatient settings, crafting personalized treatment strategies
- Implement physical therapy techniques centered on mobility, strength enhancement, balance improvement, and functional recovery
- Collaborate closely with nurses, physicians, case managers, and other rehabilitation professionals to achieve the best possible outcomes
- Inform patients and their families about therapeutic goals, discharge plans, and recommended home exercises
- Create detailed documentation of evaluations, treatment strategies, and patient progress in accordance with regulatory and facility guidelines
- Engage in care team discussions and interdisciplinary meetings to ensure seamless service delivery
- Doctorate in Physical Therapy (DPT) or a related degree from an accredited institution
- Active state licensure as a Physical Therapist
- CPR certification is preferred
- Specialized credentials (e.g., neurological, geriatric) are advantageous but not a requirement
- Prior experience is highly desirable
- Capability to care for patients with complex health and mobility challenges
- Excellent communication and teamwork abilities
- Familiarity with digital record-keeping and clinical documentation processes
