Pride Health is hiring an RN - Home Health to support our client's healthcare facility in Kissimmee, FL. Job Details: Schedule: Monday-Friday | 8:00 AM-5:00 PM | 40 Hours/Week | On-Call/Weekend: Yes Location: Kissimmee, FL; FKH coverage area Mileage reimbursed: Yes Job Type: Contract Pay Range: $52-$54 *Pay offered is based on experience, expertise, credentialing, and education.

Duties: Complete Start of Care (SOC) visits and initiate individualized patient care plans. Complete Resumption of Care (ROC) visits and reassess patient needs following changes in condition or hospitalization. Perform RN01 and RN10 visits according to established home health care plans. Perform RN11 visits and provide ongoing skilled nursing care as required. Assess patient conditions and coordinate appropriate home health services. Complete accurate and timely documentation using HCHB.

Requirements: License(s): Active RN license Certification(s): BLS Years of experience: 1+ Skills/Specialties/Technology: Home Health experience required; HCHB experience preferred.
