Health Partners of Western Ohio is an independent, non-profit and community-directed organization. We serve low-income areas and places without access to care. We're led by a volunteer Board of Directors. Most of our board members are also patients. Our Mission is to eliminate gaps in health outcomes for all members of our community by providing access to quality, affordable, preventive and primary health care.

QUALIFICATIONS
EDUCATION/CERTIFICATION: Bachelor's degree in nursing, allied health, healthcare administration or related fields.
REQUIRED KNOWLEDGE: Successful completion of in-house training.
EXPERIENCE REQUIRED: 2+ years of experience in risk management or a related role.

ESSENTIAL FUNCTIONS AND BASIC DUTIES

- Participates in and promotes health and wellness activities within the organization.
- Supports a patient-centered healthcare home where the patient-provider relationship is encouraged, with an emphasis on coordination of care and the establishment of a long-term healing relationship.
- Promotes effective and appropriate use of integrated health services at HPWO as part of a "Patient Centered Medical Home" (PCMH) model of care.
- Conducts detailed risk assessments.
- Compiles, audits, and analyzes data and information about the organization, its practices, and legal obligations.
- Reviews risk management policies and procedures.
- Records and tracks all incidents within Health Partners' organization.
- Evaluates risk levels and implications and reports findings.
- Recommends policies, process improvement activities, and plans to reduce and control risks and liabilities.
- Prepares and presents reports and proposals related to Risk Management.
- Manages risk management activities related to patient care, patient safety, and employee health.
- Attends Risk Management meetings.
- Develops quarterly risk management reports for reporting to the Quality Improvement Committee, Risk Management Committee, Board of Directors, and Administrative Team.
- Assists Chief Nursing Officer and Chief Financial Officer with liability insurance, malpractice and/or FTCA Claims.
- Assists Chief Nursing Officer and Chief Financial Officer with any items needed to complete the FTCA deeming application.
- Completes all other duties as necessary within the scope of this position as requested by the Chief Nursing Officer.
- Demonstrates effective communication skills.
- Establishes and maintains professional working relationships with other members of the health care team.
- Participates and completes ECRI risk management trainings.
- Performs other duties as assigned.
WORK ENVIRONMENT
Work is primarily sedentary, demanding sitting, walking, lifting, and bending. Those physical movements and the degree of mobility, manual dexterity, and hand-eye coordination commonly associated with duties in an office setting will be performed repetitively. This also includes bending, twisting, reaching, lifting, pulling, pushing, and walking. The ability to distinguish letters and symbols and utilize telephones, computer terminals, fax machines, and copiers is required. The work environment characteristics described here represent those an employee encounters while performing the essential functions of this job.
