What You’ll Do As a Home Health RN, you will: Perform comprehensive patient assessments and develop individualized care plans Provide skilled nursing care and ongoing patient evaluations Educate patients and families on medications, disease management, nutrition, and self-care Collaborate with physicians and interdisciplinary teams to ensure quality outcomes Monitor patient progress and communicate changes in condition promptly Supervise Home Health Aides and LPN/LVNs Maintain accurate and timely clinical documentation Deliver compassionate, high-quality care that helps patients remain safe and independent at home What You Need Current RN License Valid Driver’s License Current CPR Certification Valid Auto Insurance Preferred: 2+ years of Home Health RN experience HomeCare HomeBase experience Benefits & Perks We take care of our nurses so they can focus on taking care of patients.
