Responsibilities:

• Conduct initial dietary assessments and develop personalized nutrition plans for residents

• Collaborate with healthcare team members to ensure dietary needs are met and addressed

• Conduct regular follow-up assessments and adjust nutrition plans as needed

• Provide education and guidance to residents, families, and staff on nutrition and wellness

• Develop and implement menus and meal plans that cater to residents' dietary needs and preferences

• Monitor food intake and track resident progress

• Participate in quality improvement initiatives to continuously improve our services and outcomes
