OhioRISE, or Resilience through Integrated Systems and Excellence, is Ohio's first highly integrated care program for youth with complex behavioral health and multi-system needs.National Youth Advocate Program is proud to announce that we were selected as the Care Management Entity (CME) in Catchment Area C, made up of 11 Ohio counties: Allen, Auglaize, Champaign, Clark, Darke, Hardin, Greene, Logan, Madison, Miami, and Shelby. The CME is responsible for delivering wraparound care coordination for children and youth enrolled in OhioRISE who have moderate behavioral health needs, and for helping to grow the system of care in the communities served to ensure the behavioral health needs of children and their families are met.As a result, NYAP is seeking a fulltime CME Care Coordinator Supervisor to lead a team of Care Coordinators to cultivate flexible, family-focused, community-based responsive services based on the High Fidelity Wrap Around model of care coordination with the ultimate goal to keep youth in their homes, communities and schools by assessing and delivering the appropriate services needed and reducing unnecessary out-of-home placement and potential custody relinquishment.Working at NYAP22 Days of Paid Time Off + 11 Paid HolidaysHalf-day Fridays throughout the summerMedical, Dental, and Vision insuranceRetirement Matching (401K)Flexible HoursStudent Loan Repayment AssistanceSupervision HoursResponsibilities:• Participate in the recruitment, interviewing and training of potential new Care Coordinators.• Ensure achievement of monthly and yearly goals for staff and program.• Provide both formal and informal training for assigned staff; training should be individualized, specialized and ongoing.• Conduct performance evaluations and develop action plans for assigned staff for continued professional development Develop corrective action plans as needed.• Provide regular supervision with assigned staff to ensure open lines of communication— provide both verbal and written feedback.• Facilitates meetings with staff, at least on a bi-weekly basis or as defined by regulatory expectations, and other departmental meetings to ensure quality, holistic treatment of families and consistency in program planning.• Reviews program specific paperwork to ensure ongoing quality with documentation and around treatment planning.• Ensures adequate staff coverage and crisis coverage for clients in cases of staff absences. • Ensures development of an appropriate aftercare plan regarding ongoing client needs.• Oversee the development and implementation of all treatment and service plans for clients and their families.• Ensure program supervision is provided to staff who are making decisions about the most appropriate service for a client and family.• Ensure that staff is trained in the developmental/treatment strategies outlined in the treatment plans.• Ensure that staff is utilizing approved program strategies when working with clients and their families.• Complete written work that is legible, accurate, uses professional language, and meets established timeline.• Performs other duties as requested.Minimum Qualifications:• Will be a licensed or an unlicensed practitioner in accordance with rule 5160-27-01 of the Administrative Code, except that an Intensive Care Coordinators or Moderate Care Coordinators, care coordinator supervisor will be employed by or under contract with a Care Management Entity as described in this rule.• Have experience providing community-based services to children and youth and their families or caregivers in areas of children's behavioral health, child welfare, intellectual and developmental disabilities, juvenile justice, or a related public sector human services or behavioral health care field for: o three years with a high school diploma or equivalent; or o two years with an associate's degree or bachelor's degree; or o one year with a Master's degree or higher.• Have a background and experience in one or more of the following areas of expertise: family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development.• A supervisor that is an unlicensed practitioner will have regular supervision with a licensed practitioner and real-time access to a psychiatrist for case consultation.• Supervisors of Intensive Care Coordinators or Moderate Care Coordinators will complete the high-fidelity wraparound training program provided by an independent validation entity recognized by Ohio Department of Medicaid.• Have the qualifications and experience needed to work with children and families who are experiencing SED, trauma, co occurring behavioral health disorders and who are engaged with one or more child- serving systems. (e.g., child welfare, juvenile justice, education)• Proficient use of desktop and laptop computers, smart phones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet and database programs• Be culturally competent or responsive with training and experience necessary to manage complex cases• Live in one of the counties included in Catchment Area CDriving and Vehicle RequirementsValid driver's licenseReliable personal transportationGood driving recordMinimum automobile insurance coverage of $100,000/$300,000 bodily injury liability
