Verovian Nursing Recruitment Agency is seeking a compassionate, experienced, and highly organized Registered Nurse (RN) Care Manager to join a mission-driven care coordination team serving the greater Boston, Massachusetts area. The ideal candidate for this role is a skilled nursing professional with a strong background in eldercare, disability support, or chronic disease management. You will be responsible for delivering personalized care coordination by conducting in-home assessments, engaging with caregivers, and monitoring patient progress using both in-person and digital tools. Job specification:

• Conduct regular home visits within your assigned area to evaluate safety, assess physical and cognitive status, and monitor for any changes in health conditions.
• Review medications with patients and caregivers, provide clinical coaching, and ensure adherence to treatment plans and physician recommendations.
• Follow a personalized care plan, delivering ongoing education, coaching, and support through both in-person visits and a digital care platform.
• Build strong, trust-based relationships with patients and their caregivers, promoting engagement and collaboration in care decisions.
• Identify social, emotional, and behavioral health needs, offering referrals to community resources and services where appropriate.
• Document all assessments, interactions, care updates, and interventions in a timely and accurate manner using case management systems.
• Collaborate with primary care providers, specialists, and social workers to coordinate and streamline care delivery across multiple settings.
• Assist caregivers in reducing burnout, improving care confidence, and making informed decisions on behalf of loved ones.

Candidate requirements:

• Required: Active Registered Nurse (RN) License in the state of Massachusetts. A minimum of 2 years of recent direct care experience working with elderly adults or individuals with disabilities. Experience in case management, care planning, and patient assessment, particularly in complex medical or eldercare scenarios. Valid driver's license, active auto insurance, and willingness to travel within the assigned region. Proficiency in MS Office Suite, digital platforms, and electronic case management systems.
• Preferred: Bilingual skills are a strong asset and may impact compensation. Strong communication skills and an empathetic, patient-focused approach. Ability to work both independently and collaboratively in a team environment.
