Pay rate: $45.00/Hour (W-2)
Patient Care Setting: Home Health
Employment Type: Full-Time
Weekly Hours: 40 Hours/Week
Schedule: Monday - Friday, 8:00 AM - 4:30 PM
On-Call/Weekend Requirement: None
Productivity Expectation: 30 Points per Week
Mileage Reimbursement: Yes, mileage reimbursed between patient visits
Coverage Area: Woodford, Mercer, and Anderson Counties, Kentucky
Position Overview

• We are seeking a compassionate and dedicated Physical Therapist Assistant (PTA) to join our Home Health team serving patients throughout Woodford, Mercer, and Anderson Counties.
• The PTA will work under the direction and supervision of a licensed Physical Therapist to provide skilled therapy services in patients' homes, helping individuals improve mobility, regain independence, and achieve optimal functional outcomes.

Key Responsibilities:

• Provide skilled physical therapy treatments in accordance with the established plan of care developed by the Physical Therapist.
• Deliver high-quality patient care focused on improving strength, balance, mobility, endurance, and overall functional independence.
• Monitor patient progress and accurately document treatment outcomes and responses to therapy.
• Communicate patient status changes, concerns, and progress to the supervising Physical Therapist and interdisciplinary care team.
• Educate patients and caregivers on home exercise programs, safety precautions, and techniques to support rehabilitation goals.
• Maintain timely and accurate clinical documentation in compliance with agency, state, and federal regulations.
• Travel throughout the assigned coverage area to provide in-home therapy services.

Required Qualifications:

• Graduate of an accredited Physical Therapist Assistant program.
• Current PTA license in the state of Kentucky.
• Previous Home Health experience required.
• Current CPR/BLS certification.
• Valid driver's license, reliable transportation, and current auto insurance.
• Ability to travel throughout assigned territories.
