Justice Involved/Exceptional Needs Case Manager

ABOUT THE JOB As a member of the Complex Care team, you will provide case management services for adults in jail diversion, Aid & Assist, Community Restoration, Oregon State Hospital, civil commitment investigations, and acute care settings. Your skills in problem solving, time management, communication, crisis support, and advocacy will be essential as you partner with jail staff, team members, internal PCBH programs and community agencies to facilitate each individual’s successful transition to the community. RESPONSIBILITIES Assist individuals in obtaining supports and entitlements (Oregon Health Plan, housing, physical health, employment, substance abuse and mental health services). Coordinate care with community partners, provide service referrals, complete screenings, and develop transition plans to support engagement, retention in services and successful reentry. Accurately complete administrative tasks related to transitions and reporting; use Excel spreadsheets to track client data and coordinate referrals to residential providers. WORK SCHEDULE Monday through Friday, 8 a.m. – 5 p.m. MINIMUM QUALIFICATIONS High School Diploma or equivalent and three years of responsible and relevant professional experience; OR any equivalent combination of training, education, and experience that provides the required skills and knowledge to perform the essential functions of the job. SPECIAL QUALIFICATIONS Must possess Qualified Mental Health Associate certification from the Mental Health and Addiction Certification Board of Oregon at the time of appointment. Must hold and maintain a valid driver’s license and automobile insurance as a condition of employment. PREFERRED QUALIFICATIONS Bilingual Spanish speaking preferred (includes a 5% pay differential).
