Your Responsibilities

• Conduct thorough patient evaluations, request necessary diagnostic tests, and develop tailored care plans
• Diagnose and manage acute or chronic illnesses either independently or in collaboration with physicians, based on your credentialing
• Provide education to patients and families regarding preventive measures, wellness initiatives, and chronic disease management
• Prescribe appropriate medications and therapies adhering to your practice scope
• Engage in patient follow-up care and administer chronic disease management
• Work collaboratively within interdisciplinary teams to promote a seamless continuum of care
• Maintain accurate documentation within electronic medical records (EMR)

Qualifications

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
• Completion of an accredited Nurse Practitioner program

Necessary Certifications

• Active Nurse Practitioner license in the state of Kentucky
• National certification as a Family Nurse Practitioner (FNP-C, AANP or ANCC) or an equivalent specialty
• Certifications in Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) (or ability to procure these within 90 days)
