Job Overview A local contract Home Health Physical Therapist is needed in Riverside, California. This is a full‑time, 40‑hour per week position for 33 weeks with a pay rate of $65‑$75 per hour. About the Opportunity We are a growing home health provider serving adult patients in the community. The role involves providing skilled, physician‑directed therapy services aimed at restoring function, improving mobility, managing pain, and supporting independence in patients’ homes. Responsibilities Provide physical therapy services to adult patients in the home health setting. Evaluate patient needs and develop individualized treatment plans in accordance with physician orders. Implement therapeutic interventions to improve strength, mobility, balance, safety, and functional independence. Educate patients and caregivers on exercises, mobility strategies, and home safety recommendations. Document visits, progress, and outcomes accurately and in a timely manner. Collaborate with physicians and the interdisciplinary care team to support quality patient outcomes. Qualifications Active California Physical Therapist license. Home health experience preferred. Strong documentation and communication skills. Ability to work independently and manage a community‑based caseload. Current CPR certification and valid driver’s license preferred.
