Nurse Practitioner, Asthma Community Program (40 hours)
Position Summary/Department Summary: Nurse Practitioner, Asthma Community Program Key Responsibilities:
- Identifying patients with high-risk asthma through daily review of hospital admissions for asthma exacerbations, ED logs, chart reviews and referrals from primary care providers, asthma specialists, social workers & community partners for asthma home visits
- Working with the entire care team to determine care needs of a given patient based on his/her clinical condition & identify/steps to remove barriers and improve care
- Training and supporting Community Health Workers in conducting family-centered asthma home visits that include asthma education; assessment of patient's asthma control, medication understanding and adherence, and social needs; home environmental assessments, provision of supplies for asthma trigger reduction & case management
- Providing on-going case conferences & education related to the patient's conditions, medications & psychosocial needs
- Providing care coordination & patient advocacy across primary care, asthma specialists, community health centers, City of Boston Inspectional Services Department, Medical Legal Partnership, social services, schools & housing agencies
- Offering ongoing care to families via face-to-face meetings in the hospital inpatient units, ED & clinics, telephone outreach and follow-up, & virtual and in-person home visits
- Collaborating with community partners to advance the quality of asthma home visiting across the city & state, improve housing conditions & increase access to community resources that support the health of the community
- Managing the planning & implementation of program goals & objectives, in conjunction with Senior Management
- Facilitating relationships with external collaborators & asthma planning agencies
- Participating in external coalitions, representing & clarifying the needs & interests of the hospital & the office of community health in planning & implementation of new initiatives
- Monitoring funding & supporting grants
- Working 40 hours per week: five 8-hour shifts Monday through Friday
Education:
- Graduate from an accredited MSN/NP or DNP program.
Experience:
- 1 year of RN experience required
- 3+ years RN/NP experience preferred
- Prior experience in community health center or community programs preferred
- Familiarity with the role of Community Health Workers & patient navigators
- Spanish fluency strongly preferred
Licensure/Certifications:
- Massachusetts NP license
- MCSR and DEA upon hire
- PNP or FNP Board Certification
- BLS upon hire
- PALS certification within 3 months of hire
- A current driver's license & reliable transportation to travel to patient homes & attend meetings in Boston & MA area
Status: Full-Time
Standard Hours per Week: 40
Pay Range: $124800.00-$200262.40 Annual
