Role: LPN Primary Care professional in a tech-enabled primary care team. Responsibilities Provide direct patient care such as vital signs, injections, wound care, and point-of-care testing. Support physicians and NPs during exams and procedures. Manage triage through phone and digital communication tools. Document thoroughly in the electronic medical record (EMR). Educate patients on treatment plans and preventive care. Contribute to quality improvement initiatives and optimize workflows. Qualifications & Skills Licensed Practical Nurse (LPN) credential. Strong communication and patient education skills. Proficiency with EMR systems and digital tools. Ability to work collaboratively in a team-oriented, innovative environment. #J-18808-Ljbffr
