Overview Jackson Therapy Partners is seeking a travel Home Health Physical Therapist for a travel position in Lancaster, Ohio. This role offers a chance to help patients recover from injuries and manage chronic conditions while experiencing life in a new location. Job Details Specialty: Physical Therapist Discipline: Therapy Start Date: 04/06/2026 Duration: 13 weeks Hours per Week: 40 Shift Length: 8 hours Employment Type: Travel Minimum Requirements Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program. Active Ohio Physical Therapy license required prior to start of assignment. Current CPR certification. Valid driver's license, proof of vehicle insurance, and access to a dependable vehicle for patient visits across the service area. Experience providing home health physical therapy preferred. Ability to complete patient evaluations, develop plans of care, and document per CMS and agency requirements. Schedule flexibility to participate in weekend rotations and potential holiday availability as needed. Assignment Details Facility type: Home Health Shift name: Other Must be willing to visit patients across the agency's service area and to float to other nearby agency locations as needed.
