The LVN Home Health professional provides skilled nursing care to a diverse patient population, including elderly, post-surgery, and pediatric clients, typically managing 1-3 home visits daily. This role involves delivering comprehensive nursing services such as wound care, medication administration, vital signs monitoring, and patient education, all documented meticulously using Electronic Health Records (EHR). Reporting directly to an RN supervisor and maintaining direct client communication, the LVN plays a critical role in implementing individualized care plans and responding to patient needs in home settings.

Responsibilities

• Provide direct patient care during home visits
• Administer medications and monitor patient responses
• Monitor vital signs and conduct health assessments
• Document all care activities accurately in Electronic Health Records
• Implement and follow individualized care plans
• Educate patients and families on care procedures and health management
• Coordinate with physicians and other healthcare professionals
• Perform wound care and assist with IV therapy as needed
• Respond promptly to emergencies and changes in patient condition

Required Qualifications

• Minimum of 1 year nursing experience
• Licensed Vocational Nurse (LVN) diploma or equivalent

Preferred Qualifications

• Valid LVN license
• CPR certification
• Proficiency in basic nursing skills and medication administration
• Strong patient assessment abilities
• Experience with Electronic Health Records (EHR)
• Excellent communication and time management skills
• Empathy and critical thinking capabilities
