Job Overview:

As RN Case Manager of Luminos Hospice you will work with the Clinical Director to lead a care team including LPN(s), LVN(s), Aides, MSW and Chaplain to provide best in class hospice care.

Responsibilities

• Coordinates and oversees all aspects of patients' home care.
• Collaborates with patient care teams to develop and implement care plans.
• Educates families on patient condition, needs and care.
• Performs head to toe physical assessments.
• Checks vital signs and manages medications.
• Administers and maintains catheters.
• Complete documentation timely and accurately.
• Supervises HHAs.
• Regularly communicates patient progress to the clinical manager, physician, and care team.
• Other duties as assigned.

COMPENSATION:

This role provides a competitive base salary. Our starting range is between $75,000 - $85,000 annually. Pay is dependent on applicant's relevant experience.

Hospice Registered Nurse / RN Case Manager Requirements

• Current State RN license
• Current driver’s license
• Current CPR and BCLS certification
• Excellent verbal and written communication skills
• Minimum two (2) years' experience as an RN, hospice preferred
